Surgical resident Dr. Grace Devlin (Jordana Spiro) juggles her promising medical career with a secret deal to offer her medical services to a mob boss in order to save her ne'er-do-well younger brother's life.

With Paul Moretti out of the picture, Constantine Alexander (William Forsythe) calls on Grace (Jordana Spiro) to treat the fugitive mobster he's expecting to partner with in a shady video poker deal. Meanwhile, Grace and Brett (Zach Gilford) treat an ailing bride who's bolted for the emergency room in the middle of her wedding.

After treating a family that was involved in a fatal car accident, Grace (Jordana Spiro) is stunned when Constantine (William Forsythe) orders her to treat the hit-and-run driver's injuries, too.

After delivering a baby that Constantine (William Forsythe) is using to blackmail a politician, Grace (Jordana Spiro) tries to help the mother keep her newborn son. As Grace's treatment of an old friend uncovers a dangerous brain malady, Moretti (Michael Rapaport) looks to stop a witness from testifying at his trial.

As she fights for a critically ill woman's heart transplant, Grace (Jordana Spiro) is asked to kill her patient's estranged and imprisoned father in order to get the organ she needs to save her life.

As Constantine (William Forsythe) prepares to launch his gambling empire, Franco (James Carpinello) enlists Grace (Jordana Spiro) in Moretti's (Michael Rapaport) plan stop him. And as her unexplained absences raise questions at work, Grace sees that she won't be able to conceal her dual life much longer.

When two innocent bystanders are caught up in an ambush meant for Franco (James Carpinello), an injured Grace (Jordana Spiro) emerges from hiding in order to help. And as the war between Moretti (Michael Rapaport) and Constantine (William Forsythe) escalates, Grace must keep her brother from becoming its next victim.

Franco (James Carpinello) presses the FBI to keep him undercover to investigate Constantine's (William Forsythe) efforts to consolidate control over Chicago's competing crime families. Meanwhile, Grace (Jordana Spiro) uses her mob connections to help an old friend whose daughter is addicted to heroin.

An exiled mobster's return to Chicago draws Grace (Jordana Spiro) into the middle of a looming showdown between Constantine (William Forsythe) and undercover cop Franco (James Carpinello).

As Grace (Jordana Spiro) and Brett (Zach Gilford) treat a priest who was bitten during an exorcism, a botched collection draws Nate deeper into the mob. Meanwhile, Franco's undercover assignment to help a Detroit mobster set up shop in Chicago places him on a collision course with Constantine (William Forsythe).

As she struggles to stay out of a brewing gang war, Grace is tapped to treat Brett's estranged brother following a bicycle accident. Nate turns to his sister after he accidentally jeopardizes a mobster's valuable racehorse.

Grace is stunned to learn Constantine has an estranged son when the son makes a surprise visit to Chicago. Turns out the son is deep in debt to a drug cartel and doesn't have the money to pay them back. Now the cartel wants him dead. Constantine offers to clear the son's debt, but in order to do that, he needs help from Al Trapani. But first Trapani needs some help of his own: his girlfriend is sick and needs a doctor. Constantine assigns Grace to take care of Trapani's girlfriend and in the process she discovers a secret that could jeopardize both their lives if it ...

The episode begins right where the previous episode ended - a dangerous drug cartel has made a major move against Constantine, and Daniella was caught in the crossfire. With both her mom and Constantine fighting for their lives in Grace's hospital, Grace's mob life and her personal life have finally collided with the most tragic consequences possible. And just when things couldn't get any worse, the Feds show up at the hospital, asking questions about Grace's involvement in the Chicago mob. As the whole world collapses around her, it's up to Grace to step up and ...
